目的

探讨基质金属蛋白酶(MMP)-9,-2及其抑制因子(TIMP)-1,-2在胎膜早破(PROM) 孕妇的血浆和胎膜中的变化及与PROM的关系。

方法

选择2010年12月至2012年8月在广东省佛山市妇幼保健院妇产科诊断为PROM且行剖宫产分娩孕妇的血浆及胎膜组织35例(PROM组)和30例正常孕妇的血浆及胎膜组织(对照组)为研究对象。①采用酶联免疫吸附法(ELISA)对两组受试者血浆中MMP-9,TIMP-1,MMP-2,TIMP-2的水平进行检测。②应用免疫组织化学SP法对两组患者的胎膜组织检测MMP-9,TIMP-1,MMP-2,TIMP-2的表达(本研究遵循的程序符合广东省佛山市妇幼保健院人体试验委员会制定的伦理学标准,得到该委员会批准,分组征得受试对象的知情同意,并与之签署临床研究知情同意书)。

结果

①PROM组和对照组患者血浆中MMP-9,-2的含量比较[(4370. 43± 1078.03) pg/mL vs. (3384. 06±915. 67)pg/mL,(23 231. 7±4630. 1) pg/mL vs. (21 213. 1 ±3359. 8) pg/mL],差异有统计学意义(P<0.001,<0.05)。②PROM组和对照组胎膜组织中MMP-9,-2的阳性率比较,(100% vs. 87%,100% vs. 87%),差异有统计学意义(P<0.001,<0.001)。③PROM组血浆和胎膜组织中TIMP-1,-2的含量和表达较正常组低[(63 238. 8± 14 250. 7) pg/mL vs. (64 956. 4 ±21 171. 5) pg/mL,(1128. 1 ± 284. 4) pg/mL vs. (1146. 7±266. 4) pg/mL],两组比较,差异无统计学意义(P>0. 05)。

结论

PROM孕妇血浆中MMP-2,-9在胎膜组织中高表达可能是PROM发生的原因之一。人工诱导TIMP-1,-2或降低MMP-2,-9的含量以防止PROM的发生。

Objective

To explore the change of matrix metalloproteinase (MMP)-9, -2 and tissue inhibitors of matrix metalloproteinase (TIMP)-1,-2 in the maternal plasma and fetal membranes of pregnant women with premature rupture of membranes (PROM),as well as the relationship with PROM.

Methods

①Between December 2009 and June 2010,enzyme linked immunosorbent assay (ELLSA) was used to detect the level of MMP-9,TIMP-1,MMP-2, TIMP-2 in plasma of 35 pregnant women with PROM (PROM group) and 30 normal pregnant women (control group).② The expression of MMP-9, TIMP-1,MMP-2, TIMP-2 in fetal membranes were detected by immunohistochemistry S-P method. The study protocol was approved by the Ethical Review Board of Investigation in Human Being of Foshan Women and Children Healthcare Hospital. Informed consent was obtained from all participates.

Results

① The MMP-9 and MMP-2 level of plasma in PROM group were significantly higher than those in control group [(4370.43±1078.03) pg/mL vs. (3384. 06 ± 915. 67) pg/mL, (23231. 7 ± 4630. 1) pg/mL vs. (21213. 1 ± 3359. 8) pg/mL], with significant differences (P< 0.001, <0.05).② The positive expression of MMP-9 and MMP-2 in fetal membranes in PROM group and control group were 100 % and 87%,100% and 87%,respectively, which with significant differences (P < 0. 001, < 0. 001).③ The TIMP-1 and TIMP-2 level and expression in the plasma and fetal membranes of PROM groups were lower than those in control group [(63238. 8± 14250. 7) pg/mL vs. (64956. 4 ± 21171. 5) pg/mL, (1128. 1±284. 4) pg/mL vs. (1146. 7±266. 4) pg/mL], which with no significant differences (P>0. 05).

Conclusions

The high expression of MMP-9,-2 in fetal membranes which may be the causes of PROM, and preventive measures should be taken to prevent PROM. Artificially induced expression of TIMP-1,TIMP-2 or decreasing the content of MMP-9,-2 probably inhibits the pathogenesis of PROM.
