目的

探讨阴道分泌物预成酶谱4项,包括过氧化氢(H 2 O 2)、白细胞酯酶(LE)、唾液酸苷酶(SNA)及凝固酶(GADP),以及阴道灌洗液白细胞介素(IL)-17、-23水平,对高危型人乳头瘤病毒(HR-HPV)持续感染致病性的影响。

方法

选择2019年12月至2021年10月于内蒙古医科大学附属医院诊断为HR-HPV 持续感染,并且接受宫颈组织病理学检查的151例患者为研究对象。根据宫颈组织细胞学及病理学诊断结果,将其分为宫颈鳞状细胞癌(CSCC)组(n=30),宫颈上皮内瘤变(CIN)Ⅲ组(n=30),CIN Ⅱ组(n=31),CINⅠ组(n=29)及无宫颈病变组(n=31)。检测5组患者阴道分泌物中预成酶谱4项及阴道灌洗液中IL-17、-23水平。采用Kruskal-Wallis H 检验,对5组患者上述6项指标进行统计学比较;并采用Spearman秩相关性分析法,分析6项指标分别与HR-HPV持续感染致病性的相关关系。本研究遵循的程序符合内蒙古医科大学附属医院医学伦理委员会要求(审批文号:KY2024049)。所有受试者签署临床研究知情同意书。

结果

①5组患者阴道分泌物H 2 O 2 阳性率随着宫颈病变级别升高,逐渐下降;而LE 及SNA 阳性率与阴道灌洗液IL-17及IL-23水平,则均随着宫颈病变级别升高,逐渐升高或增加,并且差异均具有统计学意义(P<0.05)。5组患者阴道分泌物GADP 阳性率比较,差异无统计学意义(P>0.05)。②Spearman秩相关性分析显示,阴道分泌物中H 2 O2、LE、SNA 及GADP阳性率,分别与HR-HPV 持续感染致病性呈负相关关系(rs=-0.329、P<0.001),正相关关系(rs=0.365、P<0.001),无相关关系(rs=0.080、P=0.331)及正相关关系(rs=0.374、P<0.001);阴道灌洗液中IL-17、-23水平,均与HR-HPV持续感染致病性呈正相关关系(rs=0.898、0.733,均为P<0.001)。

结论

临床可通过检测阴道分泌物预成酶谱4项及阴道灌洗液免疫因子水平,对HR-HPV持续感染患者的治疗疗效、宫颈病变进展及预后进行评估。

Objective

To investigate the effects of four preformation zymograms of vaginal secretions,including hydrogen peroxide (H 2 O2),leucocyte esterase(LE),neuraminidase(SNA)and coagulase (GADP),and interleukin(IL)-17,-23 in vaginal lavage fluid on the pathogenicity of high risk human papillomavirus (HR-HPV)persistent infection.

Methods

A total of 151 patients diagnosed with HR-HPV persistent infection and underwent cervical histopathology examination in the Affiliated Hospital of Inner Mongolia Medical University from December 2019 to October 2021 were selected as the study subjects.According to the diagnosis results of cervical histocytology and pathology,they were divided into cervical squamous cell carcinoma (CSCC)group (n=30),cervical intraepithelial neoplasia(CIN)Ⅲgroup (n=30),CINⅡgroup (n=31),CINⅠgroup(n=29),and no cervical lesions group (n=31).The levels of four preformation zymograms of vaginal secretions,and the levels of IL-17 and IL-23 in vaginal lavage fluid were detected in 5 groups.Kruskal-Wallis H test was used to statistically compare the above 6 indexes in 5 groups.Spearman correlation analysis was used to analyze the correlation between the 6 indicators and the pathogenicity of HR-HPV persistent infection,respectively.The procedure followed in this study met the requirements of the Medical Ethics Committee of Affiliated Hospital of Inner Mongolia Medical University (Approval No.KY2024049).All subjects signed informed consent forms for clinical study.

Results

①The positive rate of H 2 O 2 in vaginal secretions decreased with the increase of cervical lesion grade in 5 groups.The positive rates of LE and SNA in vaginal secretions,as well as the levels of IL-17 and IL-23 in vaginal lavage fluid in 5 groups were gradually increased with the increase of cervical lesion grade,and the differences were statistically significant (P<0.05).There was no significant difference of positive rate of GADP in vaginal secretions among 5 groups(P >0.05).②Spearman correlation analysis showed that the positive rates of H 2 O2,LE,SNA and GADP in vaginal secretions were negatively correlated (r s = -0.329,P <0.001),positively correlated (r s=0.365,P<0.001),no correlation (r s=0.080,P=0.331)and positively correlated(r s=0.374,P<0.001)with the pathogenicity of HR-HPV persistent infection,respectively.The levels of IL-17 and IL-23 in vaginal lavage fluid were both positively correlated with the pathogenicity of HR-HPV persistent infection (r s=0.898,0.733,both with P <0.001).

Conclusions

The therapeutic effect of patients with HR-HPV persistent infection,the progression of cervical lesions and prognosis can be evaluated by detecting the levels of four preformation zymograms of vaginal secretions and immune factors in vaginal lavage fluid.
