目的

通过检测胎膜组织中基质金属蛋白酶-9 (MMP-9)、环氧合酶-2(COX-2) mRNA的表达,探讨MMP-9,COX-2在胎膜早破(PROM)中的表达及意义。

方法

选择2011年9月至2012年2月在本院产科住院的35例PROM孕妇为研究对象,均为初产妇。根据不同类型PROM,将其分为足月PROM(tPROM)组(n=10)和足月前(pPROM)组(n=25)。tPROM组平均年龄为(29.71±4.63)岁,平均孕周为(38.04±0.79)孕周;pPROM组患者平均年龄为(30.67±4.05)岁,平均孕周为(33.56±0.82)孕周。随机选择同期在本院住院的正常足月未破膜初产妇25例纳入对照组,平均年龄为(30.46±4.14)岁,平均孕周为(38.91±0.99)孕周(本研究遵循的程序符合本院人体试验委员会制定的伦理学标准,得到该委员会批准,分组征得受试对象的知情同意,并与之签署临床研究知情同意书)。3组受试者的年龄、孕周等一般资料比较,差异无统计学意义(P>0.05)。利用实时荧光定量PCR法测定tPROM组,pPROM组和对照组胎膜组织中MMP-9,COX-2 mRNA的表达水平。

结果

① tPROM组和pPROM组胎膜组织中MMP-9,COX-2 mRNA的表达显著高于对照组,差异有统计学意义(P<0.01),但tPROM组和pPROM组比较,差异无统计学意义(P>0.05)。② tPROM组和pPROM组患者胎膜组织中MMP-9和COX-2 mRNA的表达水平呈正相关(r=0.732,0.820,P<0.05),对照组胎膜组织中MMP-9和COX-2 mRNA的表达水平无相关性(r=0.244,P>0.05)。

结论

MMP-9,COX-2 mRNA可能相互促进,共同参与PROM的发生。

Objective

To investigate the expression and its significance of human metalloproteinase-9, cyclooxygenase-2 in membrane of premature rupture of membranes (PROM) and normal term pregnancy.

Methods

We selected 35 subjects from the pregnant women of PROM in our hospital from September 2011 to February 2012 as research group. All subjects were divided into term PROM group (tPROM group, n=10) and preterm PROM group (pPROM group, n=25). The average age of tPROM group was (29.71±4.63) years old, and the average gestational age was (38.04±0.79) weeks. The average age of pPROM group was (30.67±4.05) years old, and the average gestational age was (33.56±0.82) weeks. Meanwhile, we selected 25 subjects from the healthy term-pregnant women as control group. The average age was (30.46±4.14) years old, the average gestational age was (38.91±0.99) weeks. This study followed the procedures established by the committee meet human trials hospital ethical standards, obtained its approval. All grouping subjects were informed and consent, and signed clinical research informed consent. The messenger RNA(mRNA) of MMP-9, COX-2 in fetal membranes by using real-time PCR in different groups were examined and compared.

Results

①The mRNA expressions of MMP-9 and COX-2 in fetal membranes were significantly high in pPROM group and tPROM group than those in control group (P<0.01), however, there were no significantly statistical difference between pPROM group and tPROM group (P>0.05). ②The relationship between MMP-9 and COX-2 in pPROM group and tPROM group was positively correlated (r=0.732, 0.820, P<0.05), there was no positive correlation in control group (r=0.244, P>0.05).

Conclusions

The mRNA expressions of MMP-9 and COX-2 in fetal membranes were significantly high in PROM, and they may play key roles in the occurrence of PROM.
