盐酸氢吗啡酮(HM)是一种半合成的强效阿片类镇痛药物,主要通过激动人体μ阿片受体发挥镇痛作用。因其独特的化学结构、药效学与药代动力学作用,使其理化性质、镇痛作用及安全性和有效性,均优于吗啡,并且目前在成年患者中的临床应用较为广泛。但是,HM在小儿麻醉术后镇痛作用中的研究目前尚不充分,应用尚较少。笔者拟就HM的药效学与药代学及HM在小儿术后镇痛中的临床应用的最新研究现状进行阐述,旨在为临床了解HM在小儿术后镇痛的安全性、有效性,更好地应用于临床工作提供参考。

Hydromorphone hydrochloride (HM) is a semisynthetic potent opioid analgesic drugs, primarily plays analgesic effect through exciting μ opioid receptors. Because of its unique chemical structure, pharmacodynamics and pharmacokinetics, its physical and chemical properties, analgesic action, as well as safety and effectiveness are better than those of morphine, and is currently widely used for adults. However, research on the analgesic effects of HM in pediatric postoperative analgesia is currently insufficient, and its application is relatively limited. The authors intend to elaborate on the latest research status of the pharmacodynamics and pharmacokinetics of HM, and its clinical application in pediatric postoperative analgesia, aiming to provide references for clinicians to better understand the safety and efficacy of HM in pediatric postoperative analgesia and apply it more effectively in clinical practice.
