围绝经期焦虑患者骨科手术腰麻－硬膜外联合麻醉前的心理干预

目的

探讨围绝经期焦虑患者骨科手术腰麻－硬膜外联合麻醉(CSEA)前和术中心理干预的效果。

方法

选择2010年1月至2011年1月，在凉山州中西医结合医院和西昌力平医院采用《焦虑自评量表》确诊为ASA Ⅰ~Ⅱ级的拟行骨科手术的围绝经期焦虑患者50例为研究对象，并将其随机分为观察组(n＝25)和对照组(n＝25)。观察两组患者术前、术中和手术结束时收缩压(SBP)、舒张压(DBP)、心率(HR)和动脉血氧饱和度(SpO₂)变化(本研究遵循的程序符合凉山州中西医结合医院和西昌力平医院人体试验委员会所制定的伦理学标准，得到该委员会批准，分组征得受试对象本人的知情同意，并与之签署临床研究知情同意书)。两组患者年龄、体重指数(BMI)、围绝经期症状、麻醉方式及ASA分级等比较，差异无统计学意义(P>0.05)。

结果

观察组与对照组术前ASA评分结果显示，均存焦虑情绪，两组比较[(56.86±3.68)分vs.(58.02±3.96)分]，差异无统计学意义(t＝1.07，P>0.05)。观察组围绝经期焦虑患者术前、术中和手术结束时生命体征平稳。对照组患者SBP，DBP和HR值升高显著，与观察组比较，差异有统计学意义(P<0.05)；两组SpO₂值均较稳定，两组比较，差异无显著意义(P>0.05)。

结论

对围绝经期焦虑患者行骨科手术，CSEA前、术中给予心理干预，方法简单、实用、效果好，但是否适于拟行骨科手术的围绝经焦虑患者推广应用，尚需多中心、大样本随机对照研究进一步证实。

Objective

To investigate effects of the psychological intervention on the peri-menopausal period anxious patients with orthopedic operation before combined spinal epidural anesthesia (CSEA).

Methods

From January 2010 to January 2011, 50 peri-menopausal period anxious patients who underwent orthopedic operation were recruited into this study. All of them were taken Self-Rating Anxiety Scale (SAS) and diagnosed as SAS Ⅰ-Ⅱ in the Liangshanzhou Hospital of Integrated Traditional and Western Medicine and Xichang Liping Hospital. They were divided into two groups randomly, observation group (n=25) and control group (n=25). The following indicators were detected, systolic blood pressure (SBP), diastolic blood pressure (DBP), heart rate (HR), pulse oxygen saturation (SpO₂). The study protocol was approved by the Ethical Review Board of Investigation in Human Being of Liangshanzhou Hospital of Integrated Traditional and Western Medicine and Xichang Liping Hospital. Informed consent was obtained from all participates. There had no significance difference between two groups among age, perimenopausal symptoms and anesthesia mode(P>0.05).

Results

There had no significance difference between two groups of SAS grade(P>0.05). The vital signs of observation group was stable, and the SBP, DBP and HR of control group was increased. There had significance difference between two groups(P<0.05).

Conclusion

Psychological intervention on the peri-menopausal period anxious patients with orthopedic operation before CSEA is an simple, practical, and effective method which should be popularization and application.
