目的

探讨电子胎心监护(electronic fetal heart monitoring)检测胎心率(fetal heart rate,FHR)呈正弦型的临床意义。

方法

采用回顾性分析法分析2005年3月至2010年7月在本院产科住院分娩的孕妇中,电子胎心监护发现胎心率表现为正弦曲线波形,且病历资料完整的12例孕妇的妊娠分娩结局。(本研究遵循的程序符合本院人体试验委员会所制定的伦理学标准,得到该委员会的批准)

结果

12例孕妇电子胎心监护结果表现为正弦型中,发生在活跃期为10例(83%),活跃期前为2例(17%);胎盘形态和功能异常为6例(50%),羊水过少(羊水量<200 mL)为2例(17%)。12例新生儿中,窒息(1 min Apgar评分<7分)为3例,严重贫血(母胎输血综合征所致)为3例(25%),经及时抢救均成活。

结论

电子胎心监护呈正弦型是胎儿严重缺氧的表现,应立即抢救,这样可降低围生儿死亡率。

Objective

To explore clinical significant of electronic fetal heart monitoring resulted in a sinusoidal wave of fetal heart rate (FHR).

Methods

From March 2005 to July 2010, the clinical data of 12 pregnant women with sinusoidal fetal heart rates were analyzed retrospectively. The study protocol was approved by the Ethical Review Board of Investigation in Human Being of Pukou District Center Hospital. Informed consent was obtained form all participates.

Results

Among 12 pregnant women with sinusoidal fetal heart rate, 10 cases (83%)happened in the active stage and 2 cases(17%) before the active stage. There were 6 cases (50%) of placental abnormality, including 2 of placenta recessive early stripping, 1 of velamentous placenta, 1 of placental obstruction, and 1 of severe calcification of the placenta. In term of amniotic fluid contamination, 2 forⅠ°, 2 for Ⅱ° and 1 for Ⅲ°.And 2 cases (17%) of oligohydramnios(<200 mL). All the 12 newborns survived, including 3 cases (25%) of asphyxia (1 min Apgar score<7) and 3 cases (25%) of maternal-fetal blood transfusion syndrome.

Conclusion

Electronic fetal heart monitoring in a sinusoidal wave is a symptom of severe fetal hypoxemia, which should rescue immediately to reduce mortality.
