Endometrial cancer (EC) is one of the most common malignancies of the female reproductive system, and its incidence has been continuously rising in recent years. Patients with early-stage EC defined by the International Federation of Gynecology and Obstetrics (FIGO) clinical staging achieve a good prognosis after surgical treatment, whereas patients with advanced or recurrent EC have a 5-year overall survival (OS) rate of only approximately 17%. Traditional chemotherapy often yields unsatisfactory efficacy, and available therapeutic options remain limited to date, leaving patients with advanced and recurrent EC facing severe therapeutic challenges. Immunotherapy represents a major breakthrough in the field of oncology. In particular, the successful application of immune checkpoint inhibitor (ICI) in a variety of solid tumors has brought new hope for patients with advanced and recurrent EC. This article systematically elaborates the current research status of immunotherapy for EC, covering the application of immunotherapy in first- and second-line treatment, therapeutic efficacy, novel immunotherapeutic strategies, safety management, precise stratified treatment, and future research directions, so as to provide evidence-based reference for the clinical management of EC patients.
