To explore the clinical effects of embryo intervention, treatment of laparoscopy and transvaginal uterine in patients with cesarean scar pregnancy (CSP).

The clinical data of 32 cases of patients with CSP from January 2010 to March 2016 in Guang′an People′s Hospital, Sichuan University were selected as research subjects. According to the different methods of treatment, they were divided into laparoscopic treatment group (group A, n=10), embryo intervention treatment group (group B, n=9), vaginal surgery treatment group (group C, n=13). The transoperative bleeding volume, operation duration, length of stay, cost of hospitalization, and the duration of serum β-human chorionic gonadotropin (hCG) level resuming normal, duration of CSP diameter reducing, duration of resumption of menses and pregnancy rate after operation were statistically analyzed by statistical methods.

①There were no significant differences among 3 groups in the age, gravidity, the time interval of previous cesarean section, serum β-hCG level before operation and diameter of CSP (P>0.05). ②There were no statistically significant differences between group A and group C in the transoperative bleeding volume, operation duration, length of stay and cost of hospitalization (P>0.05), but those in group B all were better than those in group A and group C, and all the differences were statistically significant (P<0.05). ③There were no statistically significant differences between group A and group C in the duration of serum β-hCG level resuming normal, duration of CSP diameter reducing and duration of resumption of menses after operation (P>0.05), but the duration of serum β-hCG level resuming normal, duration of CSP diameter reducing after opration in group B were better than those in group A and group C, the duration of resumption of menses after opration in group B were worse than that in group A and group C, and all the differences were statistically significant (P<0.05). There were no statistical differences among 3 groups in pregnancy rate after operation (P>0.05).

Laparoscopic treatment, embryo intervention treatment and transvaginal surgery treatment have advantages and disadvantages of each. The methods of laparoscopy and transvaginal surgery can directly remove lesions, repair scar incision, and have good postoperative recovery, but both these two treatment methods need corresponding equipment and rich experience in the operation. The embryo intervention is characterized by small trauma, shorter operation time, low equipment requirement with CSP. It is especially suit for the patients with smaller gestational age, with loose endometrial and muscle layer adhesion, and with level 0, 1 and 2 of CSP grade.
