目的

观察妊娠期糖尿病(GDM)妇女母血和脐血血清内脂素水平,分析其与新生儿体重、性别比、Ponderal指数(PI)的相关性。

方法

选择2005年1月至2010年12月于牡丹江市妇女儿童医院诊治的36例GDM患者(GDM组,n= 36)为研究对象(单胎妊娠)。采用酶联免疫吸附法(ELISA)测定36例GDM孕妇和40例正常单胎妊娠妇女(对照组,n= 40)母血和脐血血清内脂素水平,并记录新生儿出生体重及身高、孕妇体重及身高,分别计算孕妇体重指数(BMI)和新生儿PI (本研究遵循的程序符合牡丹江市妇女儿童医院人体试验委员会所制定的伦理学标准,得到该委员会批准,分组征得受试对象本人的知情同意,并与之签署临床研究知情同意书)。

结果

两组孕妇年龄、孕龄、孕前平均BMI、产前平均BMI及新生儿出生体重、新生儿性别比较,差异无统计学意义(P>0.05)。GDM组新生儿PI与对照组比较,差异有统计学意义(P<0.05)。GDM组母血与脐血血清内脂素水平高于对照组,两组比较,差异有统计学意义(P< 0.05),并且母血与脐血血清内脂素水平呈正相关(r=0.342,P<0.05)。GDM组与对照组脐血血内脂素水平,均与新生儿PI呈负相关(r=-0.479, -0.336; P<0.05)。

结论

GDM组母血与脐血血清内脂素水平明显上升,并与新生儿PI呈负相关。这提示,血清内脂素可能参与胎儿的宫内发育过程。

Objective

To investigate the changes of the levels of serum visfatin in cord blood and maternal blood of gestational diabetes mellitus(GDM) women, and to analyzed the relationship among the birth weight, gender ratio and Ponderal index (PI) in GDM women's offspring.

Methods

This was a prospective observational study with institutional ethics approval and written maternal consent. A total of 36 cases of GDM were recruited in GDM group during January 2005 to December 2010. Meanwhile 40 normal pregnant women were recruited in control group. The levels of serum visfatin in maternal blood and cord blood were determined by enzyme-linked immunosorbent assay (ELISA). Birth weight and height of neonates, maternal weight and height were determined, the Ponderal index(PI) of neonates and body mass index(BMI) of pregnant women were calculated.

Results

PI of neonates had significance difference between GDM group and control group (P < 0.05); levels of serum visfatin of maternal blood and cord blood of pregnant women in GDM group were significant higher than those in control group (P<05), and their correlation was positive (r=0.342, P<0.05). Levels of serum visfatin of cord blood between GDM group and control group were negative correlation with neonatal PI (r=-0.479, -0.336; P<0.05).

Conclusion

Levels of serum visfatin of maternal blood and cord blood in GDM group were significantly increased, and had negative correlation with neonatal PI were found. Suggesting that serum visfatin may be involved in the process of fetal intrauterine growth.
