目的

观察宫颈感染人乳头瘤病毒(human papilloma virus,HPV)-16患者血清锌、白细胞介素(interleukin,IL)-2,-10水平变化,并探讨其临床意义。

方法

选取2009年3月至2010年3月,73例在本院确诊感染高危型人乳头瘤病毒(high risk human papilloma virus,HR-HPV)-16宫颈病变患者为研究对象,其中宫颈上皮内瘤变(cervical intraepithelial neoplasia,CIN)Ⅰ级为19例(CIN Ⅰ组)、CIN Ⅱ级为21例(CIN Ⅱ组)、CIN Ⅲ级为18例(CIN Ⅲ组)及宫颈癌为15例(宫颈癌组);同时,选取慢性宫颈炎患者纳入对照组(n=30),其人乳头瘤病毒检测均呈阴性 (本研究遵循的程序符合本院人体试验委员会所制定的伦理学标准,并得到该委员会批准,分组征得受试对象本人的知情同意,并与之签署临床研究知情同意书)。5组患者年龄、其他肿瘤及免疫性疾病等病史与其他内科疾病史等比较,差异无显著意义(P>0.05)。采用原子吸收光谱法测定血清锌含量,采用ELISA法测定血清白细胞介素-2,-10含量。

结果

①血清锌、白细胞介素-2含量在CIN Ⅱ组、CIN Ⅲ组及宫颈癌组之间比较,差异有显著意义(P<0.05);CIN Ⅰ组及对照组比较,差异无显著意义(P>0.05)。血清白细胞介素-10含量在CIN Ⅰ组、CIN Ⅱ组和对照组间比较,差异无显著意义(P>0.05);在CIN Ⅲ组、CIN Ⅰ组与对照组间比较,差异无显著意义(P>0.05);在CIN Ⅲ组与CIN Ⅱ组间比较,差异有显著意义(P<0.05)。②血清白细胞介素-2与血清锌含量呈正相关(r=0.69,P=0.00),血清白细胞介素-10与血清锌含量无明显相关性(r=-0.19,P=0.051)。

结论

高危型人乳头瘤病毒感染的宫颈高级别病变患者发生Th1/Th2漂移,锌缺乏可能参与该变化。

Objective

To explore changes and significance of serum levels of Zinc (Zn), interleukin (IL)-2, -10 of patients with cervical human papilloma virus (HPV)-16 infection.

Methods

From March 2009 to March 2010, 73 patients with cervical lesions and high-risk human papilloma virus (HR-HPV)-16 infection were recruited into this study, including 19 cases of cervical intraepithelial neoplasia (CIN)Ⅰ(CINⅠgroup), 21 of CIN Ⅱ(CIN Ⅱ group), 18 of CIN Ⅲ (CIN Ⅲ group)and 15 of cervical cancer(cervical cancer group). Meanwhile, 30 patients with chronic cervicitis without human papilloma virus infection were selected into control group. The study protocol was approved by the Ethical Review Board of Investigation in Human Being of Second Hospital of Tianjin Medical University. Informed consent was obtained from all participants. The serum zinc content was detected by atomic absorption spectroscopy (AAS), and the serum content of interleukin-2, -10 were detected by the method of ELISA. Ages, history of other autoimmune diseases, cancer and other medical disease, had no significant differences among 5 groups (P>0.05).

Results

①There had no significant differences of serum zinc, interleukin-2 among CIN Ⅰ, CIN Ⅱ and cervical group (P<0.05). There had no significant differences of serum zinc, interleukin-2 between CIN Ⅰ and control group (P>0.05). There had significant differences of serum interleukin-10 among CIN Ⅰ, CIN Ⅱ and control group (P>0.05). There had no significant differences of interleukin-10 between CIN Ⅰ and control group (P>0.05). There had significant differences of interleukin-10 between CIN Ⅲ and CIN Ⅱ group (P<0.05). ②Positive correlation were found between serum znic content and interleukin-2 (r=0.69, P=0.00). There had no positive correlation between serum content of interleukin-10 and znic (r=0.19, P=0.05).

Conclusion

Th1/Th2 shift in high-risk human papilloma virus infected patients with high-grade cervical lesions. Znic deficiency may be involved in this change.
