目的

探讨高功能孤独症(high functioning autism,HFA)儿童的智力结构特点。

方法

选择2006年7月至2008年7月在本院儿童发育行为中心确诊的31例高功能孤独症儿童(HFA组)及同期在广州市某小学随机抽取的45例健康正常儿童(对照组),应用修订的韦氏儿童智力量表–中国版(Wechsler intelligence scale for children–Chinese edition, C–WISC)分别对其进行测试(本研究遵循的程序符合本院人体试验委员会所制定的伦理学标准,得到该委员会批准,征得试验患儿监护人的知情同意,并与其签署临床研究知情同意书)。

结果

HFA组中,总智商(full intelligence quotient,FIQ)为低常水平的占38.71%,较对照组(11.11%)多(P=0.005)。HFA组总智商、言语智商(verbal intelligence quotient,VIQ)、操作智商(performance intelligence quotient,PIQ)及其11项分测验得分,均低于对照组(P<0.05)。HFA组的言语智商6项分测验中,数字广度和分类相对表现较好(P<0.05);操作智商5项分测验中,木块图和图形拼凑相对表现较好(P<0.05)。HFA组的言语智商与操作智商比较,差异无显著意义(P=0.413)。在HFA组言语智商与操作智商差值有显著意义的个体,所占比例高于对照组(P<0.05),且操作智商高于言语智商者多(P<0.05)。HFA组在Kaufman三因子分析中,言语理解因子(A因子)、知觉组织因子(B因子)及注意力和记忆力因子(C因子)得分,均低于对照组(P<0.001)。HFA组内三因子比较,差异无显著意义(P>0.05)。

结论

高功能孤独症儿童智力低于正常水平,发展不平衡现象突出。

Objective

To explore the profile of intelligence in children with high functioning autism (HFA).

Methods

From July 2006 to July 2008, 31 children with high functioning autism (HFA group) in the child developmental-behavioral center, Third Affiliated Hospital of SunYat-sen University and 45 normal children (control group) who were randomly selected in a school in Guangzhou were both examined by the Wechsler Intelligence Scale for Children-Chinese Edition(C-WISC). Informed consent was obtained from all participates.

Results

38.71% children in HFA group scored lower than typical developmental children on full intelligence quotient (FIQ) which was higher than that of control group(11.11%)(P=0.005). The high functioning autism children scored lower than the normal children on full intelligence quotient, verbal intelligence quotient (VIQ), performance intelligence quotient (PIQ) and all 11 subsets (P<0.05). Children in HFA group performed relatively better on digit span and similarities test in the verbal intelligence subset, and performed relatively better on block design and object assembly test in the performance intelligence subset(P<0.05). There was no significant difference between verbal intelligence quotient and performance intelligence quotient in HFA group (P=0.413). The proportion of individual in HFA group with a significant difference between verbal intelligence quotient and performance intelligence quotient was higher than that of control group, in which most children's performance intelligence quotient were higher than verbal intelligence quotient (P<0.05). The scores on the factors of verbal understand, perception organization, and attention and memory in HFA group were all significantly lower than those of control group(P<0.001), while there were no significant differences within HFA group(P>0.05) by the Kaufman three-factor model of factor analysis.

Conclusion

High functioning autism children scored lower than typical developmental children on Wechsler Intelligence Scale for Children-Chinese Edition and their intelligence structure were significantly unbalanced.
