明确成纤维细胞生长因子受体4(fibroblast growth factor receptor 4,FGFR 4)在肾脏病患儿肾组织的表达及肾脏病理发生中的作用。
采用免疫组化法观察FGFR 4在原发性肾病综合征(28例)及急性肾小球肾炎(18例)、紫癜性肾炎(20例),单纯性血尿(22例)患儿及正常肾组织(5例)的表达。分析FGFR 4表达与肾病理积分及临床指标的相关性。
各临床及病理类型肾脏疾病足细胞、近端小管、远端小管FGFR 4的表达积分均明显高于正常组(P<0.05);足细胞FGFR 4表达积分与肾小球病理积分、尿蛋白呈负相关;近端小管FGFR 4表达积分与肾小管病理积分、尿蛋白呈正相关;远端小管FGFR 4表达积分与肾小管病理积分、尿蛋白呈负相关。
FGFR 4参与肾脏病理损害的发生。不同组织的FGFR 4作用不同:对足细胞和远端小管起保护作用,而对近端小管起损害作用。
To investigate the expression and role of fibroblast growth factor receptor 4(FGFR 4)in renal tissues from the children with renal disease.
Paraffin-embedded renal biopsy tissue sections from 28 children with primary nephrotic syndrome(PNS), 18 children with acute glomerulonephritis(AGN), 20 children with Henoch-Schoenlein purpura nephritis(HSPN)and 22 children with isolated hematuria(IHU)were examined by immunohistochemical staining with anti- FGFR 4 monoclonal antibody.Normal renal tissues from 5 nephrectomized patients with nephroma were selected as control.Possible correlation between the expression of FGFR 4 and pathologic lesion score, clinical index of renal disease was analysed.
The expression of FGFR 4 in proximal tubules, distal tubules and podocytes of children with renal disease are all significantly higher than that of the control group(P< 0.05).The expression of FGFR 4 in podocytes show negative correlation with pathologic lesion score of glomerulus and proteinuria(P<0.05).The expression of FGFR 4 in proximal tubules show positive correlation with pathologic lesion score of tubules and proteinuria(P<0.05).The expression of FGFR 4 in distal tubules show negative correlation with pathologic lesion score of tubules and proteinuria(P< 0.05).
The over expression of FGFR 4 take part in the onset and development of renal disease, but the role of FGFR 4 is different in different tissues: protective factor to podcytes and distal tubules, harmful factor to proximal tubules.
