母胎医学是融合产科、遗传、新生儿、重症、影像等多领域的交叉学科,核心是将母体与胎儿作为相互依存的整体加以关注,兼顾母体安全与子代远期健康预后,已从传统降低母婴死亡率的救治模式,转向全生命周期精准健康管理。为厘清母胎医学学科的发展脉络、破解发展瓶颈,笔者拟从学科认知重构、技术理论革新、战略发展启示3方面系统梳理母胎医学研究进展趋势。①母胎医学打破儿科与妇产科学的传统学科边界,形成覆盖胚胎、胎儿、儿童至成年过渡期的连续性诊疗体系,明确母婴医学、围产医学、母胎医学3者的学科定位差异。②学科发展存在服务群体兼具健康人群与患儿群体,而且存在核心诊疗技术有限、学科与人才体系尚待完善、孕前预防体系薄弱等难题。③前沿技术与理论持续推动学科认知更新,母-胎双向交互稳态、极早产儿器官发育、人造子宫技术、全生命周期随访管理及新生儿概念界定等领域不断涌现创新性研究成果,健康与疾病的发育起源(DOHaD)、妊娠免疫机制等前沿理论,正在重塑母胎医学的传统诊疗认知。在健康中国战略背景下,母胎医学正从被动救治子代向源头预防、宫内干预、长期健康管理转型。未来该学科将依托多组学、人工智能、微创宫内治疗等技术,完善多学科协作团队(MDT)与分级诊疗体系,最终实现降低孕产妇危重症、减少重度出生缺陷、阻断孕期不良因素对子代终身损害的发展目标,完成从"保母婴存活"到"保障母子终身高质量健康"的战略升级。

Maternal-fetal medicine is an interdisciplinary field integrating obstetrics, genetics, neonatology, critical care, imaging, and other related specialties. Its core concept is to consider the mother and fetus as an interconnected unit, with a focus on maternal safety and long-term health outcomes of the offspring. The field is evolving from a traditional care model focused on reducing maternal and perinatal mortality toward life-course precision health management. To clarify the developmental trajectory of maternal-fetal medicine and explore current challenges, the author summarizes research advances and future trends in maternal-fetal medicine from three perspectives: redefinition of disciplinary concepts, innovation in technologies and theories, and implications for strategic development. ① Maternal-fetal medicine has expanded beyond the traditional boundaries between pediatrics and obstetrics and gynecology, establishing a continuous care framework spanning embryonic development, fetal life, childhood, and transition to adulthood, while clarifying the distinct disciplinary positioning of maternal-child medicine, perinatal medicine, and maternal-fetal medicine. ② The development of maternal-fetal medicine faces several challenges, including the coexistence of healthy populations and patient populations among its target groups, incomplete development of key diagnostic and therapeutic technologies, incomplete disciplinary and workforce development systems, and insufficient preconception prevention strategies. ③ Advances in emerging technologies and theories continue to reshape the understanding of maternal-fetal medicine. Recent research advances have emerged in areas including maternal-fetal bidirectional interactions, organ development in extremely preterm infants, artificial womb technology, life-course follow-up and health management, and the redefinition of neonatal concepts. Meanwhile, frontier theories such as the developmental origins of health and disease (DOHaD) and pregnancy immune mechanisms are reshaping traditional concepts of maternal-fetal care. Under the framework of the Healthy China strategy, maternal-fetal medicine is transitioning from passive rescue of offspring toward early prevention, intrauterine intervention, and long-term health management. In the future, the field will rely on advances in multi-omics, artificial intelligence, and minimally invasive fetal therapy, while improving multidisciplinary team(MDT) and tiered healthcare systems, ultimately achieving the strategic transformation from " ensuring maternal and infant survival" to " promoting lifelong high-quality health for mothers and offspring" .
