腹腔镜在卵巢癌诊断、评估及初始治疗决策中的应用发展迅速。合理应用腹腔镜技术对早期卵巢癌患者进行分期手术,对手术后诊断的卵巢癌患者进行腹腔镜下补充分期手术,或对晚期卵巢癌患者进行腹腔镜下肿瘤细胞减灭术的可行性评估和分层治疗,均已得到学术界的广泛认可。腹腔镜下晚期卵巢癌肿瘤细胞减灭术难度较大,临床应用目前仍颇受质疑。笔者拟围绕晚期卵巢癌腹腔镜下肿瘤细胞减灭术的适应证、术前评估、手术技巧、安全性等相关问题进行详细阐述,以资借鉴。

The applications of laparoscopy in the diagnosis, evaluation and primary treatment decisions of ovarian cancer is developing rapidly. Laparoscopic staging surgery for apparent early stage ovarian cancer, re-staging surgery for post-operative diagnosed ovarian cancer, and laparoscopic prediction for optimal cytoreductive surgery in advanced ovarian cancer are reasonable and being widely accepted by academia. Laparoscopic cytoreductive surgery for advanced ovarian cancer in clinical is very difficult and still under controversy. We will focus on the indications, preoperative assessment, surgical skills and the safety of laparoscopic cytoreduction for advanced ovarian cancer, in order to provide references for clinicians.
