妊娠期肝内胆汁淤积症(ICP)是产科较常见疾病,对胎儿具有严重不良影响。ICP病因和发病机制迄今尚未明确,免疫因素被认为参与其发病。辅助性T细胞(Th)中,Th1/Th2免疫平衡向Th1偏移是ICP发病的重要环节。Th17分泌过量白细胞介素(IL)-17,巨噬细胞分泌大量肿瘤坏死因子(TNF)-α,调节性T细胞(Treg)细胞功能及人类白细胞抗原(HLA)表达水平的下调,以及其与其他细胞因子相互作用均可导致其发病。笔者就免疫系统与ICP的发生发展的关系进行综述如下。

Intrahepatic cholestasis of pregnancy(ICP) is a common obstetric disease. It has adverse effects on the fetus. Its etiology and pathogenesis are unclear now. Immune factors are thought to participate in pathogenesis of ICP. In T helper cells (Th), the immune balance of Th1/Th2 shifted towards Th1 is an important part of the pathogenesis of ICP. Th17 secreting excess interleukin(IL)-17, macrophages secreting a lot of tumor necrosis factor (TNF)-α, lower function of regulatory T cells(Treg), lower expression of human leukocyte antigen (HLA) and interacting with other cytokines lead to its pathogenesis. The relationship between the development of ICP and the immune system will be summarized.
