川崎病并发冠状动脉损伤的危险因素分析

目的

探讨川崎病并发冠状动脉损伤(CAL)的相关危险因素。

方法

选择2011年5月至2014年4月于四川大学华西第二医院住院治疗的491例川崎病患儿的临床资料为研究对象,并按照心脏彩色多普勒超声检查结果将其分为CAL组(川崎病并发CAL,n=52)和NCAL组(川崎病未并发CAL,n=439)。对其性别、年龄、临床诊断、红细胞沉降率、血小板计数、血红蛋白、C反应蛋白(CRP)、外周血白细胞计数、静脉注射用免疫球蛋白(IVIG)开始使用时间及发热持续时间进行单因素分析及多因素logistic回归分析。本研究遵循的程序符合四川大学华西第二医院人体试验委员会所制定的伦理学标准,得到该委员会批准,分组征得受试对象监护人的知情同意,并与之签署临床研究知情同意书。

结果

两组患儿性别、红细胞沉降率、血小板计数、血红蛋白、IVIG开始使用时间及发热持续时间比较,差异有统计学意义(P<0.05)。多因素logistic回归分析结果显示,性别、红细胞沉降率及IVIG开始使用时间是川崎病并发冠状动脉扩张的高危因素(P<0.05)。

结论

男性患儿、红细胞沉降率升高及延迟使用IVIG治疗是川崎病并发CAL的高危因素。

Objective

To analyze risk factors of coronary artery lesions(CAL) in children with Kawasaki disease.

Methods

From May 2011 to April 2014, a total of 491 children with Kawasaki disease in the West China Second University hospital were included in the study.They were divided into CAL group(CAL caused by Kawasaki disease, n=52) and NCAL group(Kawasaki disease only, n=439). Risk factors potentially associated with CAL including gender, clinical diagnosis, age, erythrocyte sedimentation rate(ESR), platelet count, hemoglobin, C reactive protein(CRP), white blood cell count, starting time of intravenous immunoglobulin(IVIG), duration of fever were collected. The univariate analysis and multivariate logistic regression analysis were performed.The study protocol was approved by the Ethical Board of West China Second University Hospital.Informed consent was obtained from each participants' parents.

Results

There were significant differences in gender, ESR, platelet count, starting time of IVIG and duration of fever between two groups(P<0.05). Multivariate logistic regression analysis showed gender, ESR and starting time of IVIG were independent risk factors of CAL in children with Kawasaki disease.

Conclusions

Male, increased ESR and delayed IVIG treatment were the high risk factors of CAL caused by Kawasaki disease.
