目的

探讨视听整合持续作业测试(integrated visual and audio continuous performance test,IVA-CPT)在注意缺陷多动障碍(attention deficit hyperactivity disorder, ADHD)诊断中的作用。

方法

本研究以2007年5月至2007年11月在本院确诊为注意缺陷多动障碍的60例(病例组)和有相似主诉,但排除注意缺陷多动障碍的41例(对照组)(分组征得患儿家属的知情同意),共计101例为研究对象。同时,进行视听整合持续作业测试,比较两组结果。

结果

在视听整合持续作业测试判断标准第1、第2条中用到的参数:综合尺度反应控制商数、综合尺度注意力商数、听觉反应控制商数、视觉反应控制商数、听觉注意力商数、视觉注意力商数、多动商数、理解力商数,两组比较,病例组均显著低于对照组,差异有显著意义。按照视听整合持续作业测试全部3条标准判断结果,诊断注意缺陷多动障碍的灵敏度为78.33%,特异度为56.10%;按照其中第1、第2条标准判断结果,灵敏度为73.33%,特异度为75.61%。

结论

视听整合持续作业测试的部分参数:综合尺度反应控制商数、综合尺度注意力商数、听觉反应控制商数、视觉反应控制商数、听觉注意力商数、视觉注意力商数、多动商数、理解力商数,能有效区分注意缺陷多动障碍和有相似症状而非注意缺陷多动障碍的儿童,其第1、第2条判断标准与临床诊断有较高一致性,参考价值较好。

Objective

To investigate the effect of integrated visual and audio continuous performance test(IVA-CPT) on the diagnosis of attention deficit hyperactivity disorder (ADHD).

Methods

From May 2007 to November 2007, children with alike symptoms of attention deficit and/or hyperactivity disorder children were divided into the test group and the control group(treatment in West China Second University Hospital, Sichuan University). The test group was diagnosed as attention deficit hyperactivity disorder according to the standard of Diagnostic and Statistical Manual of Mental Disorders-Ⅳ. The suspected subjects look-alike symptoms but not attention deficit hyperactivity disorder were the control group. Both of them were accepted to the integrated visual and audio continuous performance test.

Results

The parameters including full scale response control quotient(FRCQ), full scale attention quotient(FAQ), auditory response control quotient(ARCQ), visual response control quotient(VRCQ), auditory attention quotient(AAQ), visual attention quotient(VAQ), hyperactivity quotient(HYP), comprehension quotient(COM) of integrated visual and audio continuous performance test of attention deficit hyperactivity disorder group (the test group) were significantly lower than those of the control group. If judged by all that 3 judgment items of integrated visual and audio continuous performance test, the sensitivity was 78.33%, the specificity was 56.10%. If judged by the first and the second item of judgments, the sensitivity was 73.33%, the specificity was 75.61%.

Conclusion

The parameters including full scale response control quotient(FRCQ), full scale attention quotient(FAQ), auditory response control quotient(ARCQ), visual response control quotient(VRCQ), auditory attention quotient(AAQ), visual attention quotient(VAQ), hyperactivity quotient(HYP), comprehension quotient(COM)of integrated visual and audio continuous performance test can effectively differentiate children with or without attention deficit hyperactivity disorder. The first and second item of judgments are highly accordance with clinic diagnosis, which would be valuated as a reference.
